Job Description:
Under the direction of the Program Manager II– Patient &
Community Wellness, the Health Educator will be responsible for the
implementation of all initiative deliverables. The Health Educator
will provide Nutrition and Physical Activity Education, assist in
implementing food insecurity screenings at targeted sites, lead,
conduct formative assessments, documentation via NEVHC and
Department of Public Health (DPH) database, and collaborate with
DPH staff to complete the IOE plan, participate in outreach
activities, identify, screen and enroll patients into the Produce
Prescription Program (Fresh and Healthy) and lead other
deliverables under this program, and participate in the maintenance
of existing edible gardens. The Health Educator will also
collaborate with project and department staff to organize food
distribution events paired with taste-tests and nutrition education
at targeted and partner sites. The Health Educator will participate
in DPH approved community events and work with DPH to conduct
on-going evaluation to refine training as needed. The Health
Educator will maintain documents that support PSE implementation,
evaluation, and promotion. Grant Cycle Period: 04/01/26 through
02/28/27 REPORTS TO : Program Manager II - Patient & Community
Wellness POSITION REQUIREMENTS: 1. Bachelor’s degree in Nutrition
Science, Public Health, Health Education, Kinesiology, or related
field. 2. A background in nutrition, physical activity, and
gardening. 3. Experience in conducting food demonstrations in
underserved communities. 4. Good interpersonal skills and ability
to present in front of a group. 5. Must be able to relate to a
variety of individuals from all walks of life, including from
ethnically, gender, sexual orientation and age, diverse
backgrounds. Ability to work with populations impacted by HIV (gay,
males, IDUS, etc.). 6. Computer skills in Microsoft Office programs
(Word, Excel, etc.), and database systems. 7. Ability to be
flexible and work in a changing environm 8. Ability to work
evenings and weekends is required. 9. Sensitivity to the different
cultures represented among members and staff. 10. Demonstrated
ability to listen and communicate with others in a professional and
caring manner including sensitivity with individuals from diverse
cultures and lifestyles. 11. Demonstrated ability to set priorities
for tasks to work effectively in spite of interruptions and under
minimal supervision. Self-starter, reliable and dependable. 12.
Fluency in English (speaks, read and write) is required. 13.
Fluency in Spanish (speaks, translate, read and write) is required.
14. Ability to maintain absolute confidentiality about health care
and other patient/client information. 15. Current California
driver’s license, appropriate insurance coverage and a driving
record acceptable to the NEVHC’s insurance carrier (if required to
