Services is an experienced licensed and certified Social Worker
(LCSW) responsible for excellence in the clinical practice of
Social Work team and the management of patient care in the hospital
and emergency room. Reporting to the Senior Vice President of
Population Health, the Manager of Social Services has 24 hour
accountability for professional practice in Social Work and its
impact on planning the patient's progression and transition of care
and the activities associated with assuring assessment and
advocacy. The Manager of Social Services must demonstrate the
ability to recognize and respond to the unique needs of each
patient according to the Hospital's Age and Population Specific
awareness. This role encompasses the development of professional
practice and staff competency through assessment and education,
compliance with regulatory and professional standards, and a
commitment to contribute to improved clinical outcomes, service
levels, patient experiences, safety, and collaborative
relationships. This position requires the full understanding and
active participation in fulfilling the Mission of Martin Luther
King, Jr. Community Hospital. It is expected that the employee will
demonstrate behavior consistent with the Core Values. The employee
shall support Martin Luther King, Jr. Community Hospital's
strategic plan and the goals and direction of the quality and
performance improvement process activities. ESSENTIAL DUTIES AND
RESPONSIBILITIES Demonstrates positive interpersonal relationships
in dealing with peers, physicians, and other customers to promote
an efficient teamwork and quality patient care: interpersonal
complaints are remedied in a mature fashion; works at establishing
good rapport and professional working relationships with all
department staff; maintains appropriate interdepartmental
communications; able to identify interdepartmental problems and
resolve them promptly at the manager level. Participates in
hospital committees and unit specific multidisciplinary
conferences, as well as unit staff meetings: distributes staff
meeting minutes in a timely manner. Implements unit specific
Quality and Performance Improvement Plan, completes monitoring
forms and reports findings to Senior Vice President of Population
Health and Quality and Performance Improvement Committee. Models
leadership and promotes professionalism to assist staff in
developing the following: priority setting; team work; flexibility;
coordination of activities; organizational skills; delegation; and
behaviors consistent with MLKCH Mission & Values. Facilitates the
development of professional Social Worker practice within the
Hospital. Develops and implements efforts to assure the
identification of at-risk patients and promote timely intervention
and advocacy for those patients. Assures compliance with reporting
requirements for CDPH. Completes schedule, assuring adequate
coverage at all times to meet regulatory requirements. Maintains
standard of care by monitoring staff activity and adjusting
assignments as needed: monitors orientee's activities and assists
preceptors in enhancing orientee's experience; assist with
difficult orientation situations and/or assignment needs; delegates
care management activities to ensure they are completed by the end
of the shift; monitors patient care and provides feedback to staff
members and providers related to patient progress to treatment
modalities related to timely discharges as needed. Holds staff
accountable for the care management of assigned patients; ensures
staff education; confers with staff and makes rounds on patients
during the shift and as needed; audits social work documentation
for compliance; and utilizes data to assess effectiveness of
departmental planning efforts. Per unit guidelines and for
accuracy; utilizes professional judgment and data to make staffing
decisions; and communicates staffing decisions to the team.
Participates in hiring process, which may include: reviewing
applications and interviewing candidates; including staff, as
appropriate; completing appropriate interview documentation; making
recommendations regarding staffing needs / skill mix. Counsels
staff members as indicated by performance or behavior: utilizes
steps in progressive discipline in collaboration with Human
Resources; documents according to prescribed procedures and
maintains an objective relationship with staff; issues are promptly
identified and staff counseled during same shift or next working
shift; Human Resources is appraised of all staff counseling.
Provides supervision for MSW staff seeking their LCSW. Collaborate
with hospital leadership and IT team to develop1 documentation and
workflow for all teams. Manage budgets for assigned cost centers
and meet with finance department as needed. Oversight of MSW
Internship program and Substance Use Navigator staff. Evaluates
employees according to policy and within one month of the due date.
Works with staff to determine realistic goals and meets regularly
with staff throughout the year to provide constructive feedback and
direction. Assures accurate documentation of employee's progress
against goals and care standards. Promotes staff compliance with
hospital safety and emergency procedures on a daily basis. Guides
staff in investigation and completion of variance reports and
forwards completed reports to Senior Vice President of Population
Health. Review Payroll in UKG: monitors overtime; monitors usage of
agency personnel; monitors tardiness and absenteeism. Maintains
intimate knowledge of Care Management Department policies and
procedures and acts as a resource for the RN's in the department.
Responsible for updating and reviewing policies pertaining to
Social Services. Represent Social Work on the Bioethics Committee
and Unidentified Patient Subcommittee. Facilitate monthly staff
meetings. POSITION REQUIREMENTS A. Education/Licensure Master's in
social Worker License in Clinical Social Work required Current
license to practice Social Work in California. B.
Qualifications/Experience Knowledge and understanding of clinical
Social Work and mental health counseling principles, methods,
procedures and standards. Able to navigate and connect successfully
with outside provider networks (Healthplan's, IPAs, FQHC's) Minimum
of five (5) years of clinical Social Work experience and
familiarity with standards of Social Work practice and regulatory
guidelines. Minimum of two (2) years leadership experience
